Answer A: Were V:tM rules intentionally badly designed?
Not sure how to answer this beyond saying I ran a V:tM campaign for six years after it came out, and the rules worked out fine for me. I wasn't super strict, perhaps, but the basic dice mechanic worked. I actually like it better than the ubiquitous single-roll style of D&D.

Answer A: Did Mentzer and Gygax play together?
I can confirm that they played board games together. Mentzer is a fan of Ticket to Ride, and used to play it with Gygax. (This is per a conversation I had with Mentzer.) Doesn't quite answer your question, but does confirm they gamed together in some form.
